An Introduction to Advance Care Planning
How prepared are you? End-of-life planning is for everyone, and whether this is something you haven’t begun thinking about or something you have had completed for years, the End of Life Care Partnership anchored at United Way of Tucson is here to educate and support you. It is the Partnership’s mission to enhance the way we live by fundamentally changing the way we talk about death, and we believe these conversations can begin at home and in the workplace. This series of videos will give you an introduction to end-of-life planning to get you started on having your own conversations.
Please follow the steps below to learn more about Advance Care Planning and fill out your own Advance Care Directives to ensure your wishes are fulfilled.

Step Five: Download and fill out a copy of 5 Wishes Advance Care Directive
Our Partnership recommends using Five Wishes for your advance care planning for its ease of use, and for the way it guides users through the process. To access other versions of Advance Care Directives, please visit our website here.
