End-of-life decision-making may have complex physical, emotional, spiritual, and financial impacts for the individual and the caregiver. Each person’s situation is unique given their values, needs, fears, and hopes for what they want at the end of their life. This session goes beyond advance directives and what is traditionally known as advance care planning. We’ll discuss some of the communication barriers and focus on specific strategies that we can use to embrace authentic person-centered care firmly grounded in the informed choice that reflects the individual’s goals and wishes.
