Five Wishes lets you say exactly how you wish to be treated if you get seriously ill. It was written with the help of the nation’s leading experts in end-of-life care and is easy to use. All you have to do it check a box, circle a direction, or write a few sentences.

You can order your own Five Wishes Advance Directive and help our Partnership at the same time. By purchasing Five Wishes through the links below, a portion of the proceeds benefit the work of the Arizona End of Life Care Partnership. This allows us to continue providing education, services, and resources to people across Tucson and Arizona.
